Dental Implant Records: What to Take Home After Treatment

Before leaving Antalya, ask for your implant system details, component records, treatment summary, images and follow-up plan. Use this patient checklist.

Before leaving Antalya after dental implant treatment, ask your treating provider for a written treatment summary and the details of the implants and restorations used. Keep those records with your aftercare instructions and bring them to future dental appointments. A brand name alone is less useful than a record identifying the actual system and components.

What should I ask the treating provider to record?

Use this checklist to request the information relevant to your treatment. Ask the provider to explain any item that does not apply or is not yet available.

RecordDetails to request
Treatment summaryTreatment dates, what was completed, the teeth or implant positions involved and any work still planned
Implant identificationManufacturer, implant system or model, product reference and lot or batch number for each implant
Implant dimensionsDiameter and length, recorded against the correct implant position
Abutments and other componentsThe component type and identifying details available for the abutments, attachments and relevant parts used
Crown, bridge or dentureWhether it is temporary or final, its material and the restoration details the treating provider can supply
Images and clinical recordsCopies of relevant X-rays or other records, with an explanation of how another dentist can request further information
Follow-up planReview dates, individual care instructions, the clinical contact route and arrangements for any unfinished stage
Written cover and paymentsYour agreed treatment plan, invoices and the applicable warranty terms

What is an implant passport?

An implant passport is a convenient record of implant and restoration details. Manufacturer formats vary. For example, Straumann's patient passport includes product reference and lot numbers, implant dimensions, placement dates and abutment details. This is an example of a record format; it does not identify which system is suitable for you or promise that a particular brand is included in your quote. [1]

Ask whether the provider uses a passport, component labels or another written record. Check that each entry clearly relates to the correct implant position. Keep a secure digital copy as well as any original paperwork.

Can a product number confirm everything about my treatment?

Product identifiers help with traceability. Some manufacturers also provide verification tools; Straumann, for example, uses a product reference and lot number for its online component check. A product check does not assess the quality of your diagnosis, surgery or restoration, and it does not establish that another provider can undertake a particular repair. [2]

If a record is missing, request it from the treating provider rather than guessing the system from a photograph or brochure. Share the records directly with a dentist involved in your care through an agreed private channel.

Do records replace aftercare or warranty terms?

No. Keep the component information alongside your individual follow-up plan and written warranty. Component cover and treatment cover are different, and an implant crown is excluded from the lifetime component cover described in New Look Smile Group's current guidance. Treatment warranty is 2–5 years depending on the treatment. Implant, abutment and attachment components may have lifetime cover under the applicable terms. Six-month dental check-ups and proof of attendance are required for warranty eligibility.

Ask what applies to your own case, including any costs for assessment, further treatment, accommodation or travel. See the aftercare and warranty checklist.

How does this fit into my Antalya treatment plan?

Use the dental implants in Antalya, Turkey page for treatment enquiries and the implant trips and timeline guide for planning questions. Before each trip, ask when the records for that stage will be supplied. Our team can help coordinate questions with the treating provider through the contact page.
